Please call 608-835-3388 to schedule an appointment or email us at smile@drgardocki.com, our office hours are Monday thru Thursday from 8:30am to 5:00pm and Fridays 8:30am to 2:00pm. If you reach our office after hours you may leave your name and telephone number and we will return your call the next business day.

If you have pain or an emergency situation, every attempt will be made to see you that day. If you have an after-hours emergency, you can reach our doctors by calling the cell phone number that is available on the answering machine.

Cancellation and No Show Policy

Please remember to cancel your appointment at least 24 hours in advance to avoid a cancellation fee.
